Every patient at Western Pennsylvania Sports Medicine & Rehabilitation Clinic begins their care with a comprehensive initial injury evaluation to guide a personalized therapy plan. One of our experienced professionals will evaluate your condition and recommend the appropriate course of treatment. Our licensed physical therapists and athletic trainers assess thousands of injuries each year.

What To Expect

When to Arrive

Your first visit will last about an hour. Please arrive 15 minutes early to sign in, complete any paperwork, and change clothes if needed.

What to Bring

  • Physician referral or prescription (if required)
  • Insurance card
  • Primary registration forms
  • Photo ID or driver’s license
  • Co-payment (if applicable)
  • A change of clothes (optional)

How It Works

During your first visit, your physical therapist will complete an initial evaluation and discuss a plan of care with you. This evaluation helps set treatment goals based on your needs. These may include improving movement, strength, flexibility, and endurance while reducing pain. Future visits will be guided by your diagnosis and individualized goals.

Starting Your Therapy Program

Whether you’re new to therapy or returning for continued care, our team is here to help you through every step. From insurance verification to appointment scheduling and billing, we’ll support you throughout your experience.

New Patient Forms

Forms can be completed through our secure Patient Portal, to which you’ll receive a secure link via email after you set up your first appointment, or when you arrive at your first appointment. To access the portal, schedule your appointment and request a Welcome Letter to be emailed to you. If you’re unable to complete the forms in advance, plan to arrive 15 minutes early to fill them out in the office.

What to Wear

Wear comfortable clothing that allows for easy movement. Athletic wear and gym shoes are ideal.

Your First Appointment

You’ll be greeted by our Patient Care Coordinator, who will collect your ID, insurance card, and any paperwork you’ve completed. If your forms aren’t finished, they’ll provide them for you to complete before meeting with your therapist.

Once you’re checked in, your physical therapist will bring you to a private exam room for a comprehensive evaluation. They’ll review your medical history, perform any necessary tests, and talk with you to better understand your condition.
